Incident Safety Officer - Fire Suppression Certification Application Fire Department Safety Officers Certification System Who may apply Members of the Fire Department Safety Officers Association, applicants sponsored by the Authority Having Jurisdiction (AHJ) and individuals meeting the duties and requirements of a company officer or an Incident Scene Safety Officer. Requirements Applicants shall meet the requirements of NFPA 1521 – 2008 ed. Chapter 4 Section 4.5.1 and be a member of a fire department an Emergency Services Organization with at least 5 years experience. Study Materials Fire Department Incident Safety Officer, second edition, David W. Dodson, Delmar/Cengage Learning, 2007. NFPA 1521, Standard for Fire Department Safety Officer, 2008 edition. Ways to Test Individualized Testing: The exam is administered to the certification candidates at their location utilizing a local proctor. Testing materials are sent to the proctor and the candidate reports to the proctor for testing. Departmental Testing: The exam is administered to two or more certification candidates from the same emergency service organization at the organization's location utilizing a local proctor. Testing materials are sent to the proctor and the candidates report to the test site for testing. Regional Testing: The exam is administered to two or more certification candidates from different emergency service organization at a host organization's location utilizing a local proctor. Testing materials are sent to the proctor and the candidates report to the host organization's test site for testing. Host organizations are responsible for providing a room for testing and promoting the exam. The dates, times and locations are listed on the Events Page. Conference Testing: The exam is administered in conjunction with national conferences such as FDSOA Annual Conference, FDSOA Apparatus Symposium, FDIC and Fire Rescue International. Examination Procedure The examination will be a 100 question multiple-choice test, which measures your knowledge, abilities and aptitudes to perform an Incident Safety Officer assignment. You will be notified, by mail, when and where to appear. Please bring a valid picture ID with you. Four (4) hours will be allowed to complete the test. Registration begins 1/2 hour before exam. Applicants not checked in 15 minutes before start of exam will be unable to take the exam. We will mail your score to you, but we cannot tell you your ranking in relationship to other applicants. Application Deadline Completed applications with payment must be in the FDSOA Offices 15 days before exam date. Application will not be processed until payment is received. Payment Must Accompany Registration Form Non-Members: $195.00 FDSOA Members: $95.00 Save $15.00 by signing up as a member for an additional $85.00. Membership runs 12 months from date payment is received. Refunds A full refund will be given the applicant (or sponsoring organization) provided that written notification is received by FDSOA, at least one (1) week prior to exam.
