Certification Registry

Over the last three years, FDSOA has been gathering the last four digits of an FDSOA Certified Safety Officer's Social Security Number or Social Insurance Number for Canadian residence. The purpose was to create a webpage for FDSOA Certified Safety Officers to view their Certification level and most importantly to see their Certification Expiration date in order to re-certify. We are pleased that we can make the new webpage available to you.

To view your personnel information please login by entering your last name and the last four digits of your Social Security Number or the last four digits of your Social Insurance Number.

If you cannot login, the problem may be as follows:

• Your Certification has expired and you have not re-certified within the last five years.
• FDSOA does not have the last four digits of your Social Security Number or your Social Insurance Number in our database. Please email FDSOA's Certification Coordinator at programs@fdsoa.org with the following information: Your First and Last Name followed by the last four digits of your Social Security Number or the last four digits of your Social Insurance Number.
• The database is not current. The database is updated on the first business day of a new month. If you were certified on the 15th day of the month, your name will not appear on the certification registry until the 1st day of the new month.
